It's because some people on LW know each other in real life, and when they are curious about strange bumps on their pen0r or they want to learn about drugs they may not their friends on LW to know. Also, people care about their internet ego often, so this is also applicable to online friends and not just in real life friends.
Seems pretty straight forward for me.

Then everyone would get second accounts that are only used once or twice. That sort of promotes idiocy in itself, unless you're willing to pay for the server that hosts all the different account information.
