I used this one as a template, because I thought it was very eye catching (I've gotten some great results from it, and every interview that I've gone to with it has lead my interviewer to tell me "wow, that's a great looking resume. How'd you do that?!" Generally, I tend to put this information:

If your resume is a bit short, I'd put reference at the bottom like:
