I think the artist meant the figure to represent either sex. Otherwise, he would have given the figure some physical characteristic to define it as either male or female.

Maybe people think that it looks like a women but in I think it is a self portrait. If you look at the context of when he painted it was right before his mental break down. I really think it was how he was feeling about himself at the time and thats how he expressed it.
