I would say terminal disease. The whole time you're going through life, you know that you're going to die; you just don't know when.

When it comes to a terminal disease, there are things that can be done to make your last days more comfortable and make the dying process less frightening and less painful (particularly because you know it's coming, so that you can prepare for it). Murder, depending on how it's done, can be quick and painless, and while not exactly lacking scariness, it's certainly not as terrifying to me as being lit on fire or slowly having your lungs fill with water.
