As for what happens to the cocaine, it continues merrily on in circulation until it is broken down, undoubtedly in the liver, and what is left of it is filtered out of circulation in the kidney, and then you piss it out.

Physically, it BLOCKS the molecular machinery that brings it back into the cell. A more specific answer is not needed, and nor can it be given without producing a 3-dimensional image (perhaps timelapse) of precisely how the two tertiary structures interact.

yeah I know stuff gets blocked, but how is the cocaine doing this?! Are there little pieces of powdered cocaine sitting in the doorway of the dopamine receptors? Like literally the powder is sitting there and dopamine is bumping into a big wall of cocaine it can't get through?

I've seen videos of how antidepressants work but I get the sense they're making shit up as I can't imagine serotonin bumping into a wall of paxil and getting all  butthurt because it can't get in the receptor cos the pieces of paxil are so fat

Meet cocaine.

Due to its structure, it is able to interact in such a way with the dopamine transport molecule so as to render it inoperable. How, specifically? Either a molecule fits into the spot normally occupied by dopamine to be transported and prevents dopamine from binding (this is known as competitive inhibition) or it induces some sort of conformational change which renders it extremely difficult or impossible for dopamine to bind (non-competitive inhibition).

Do I know or particularly care which of the two is happening here? Not especially. You can do your own research.

note: SSRI's like Paxil would work like this, but MAOI's have a different and not strictly analagous method of action.

Cocaine can bind to the receptors on the post synaptic membrane (like a door). It prevents the dopamine, adrenaline, etc, from accessing this door and hence cannot pass into the other side, so they remain in the synaptic cleft. If this prevents enough neurotransmitter from travelling across the synaptic cleft it can prevent the opening of the sodium ion channels, preventing an action potential because the minimum voltage is not met, and hence the impulse is not propagated.

Have you never heard of the concept of enzymes, active sites, induce-fit theory? All this is pretty understandable if you know that, and that's not really high level stuff.
