He is suffering probably from phimosis. It's pretty unusual but it happens. He should go to the doctor if stretching is not working. The doctor may prescribe creams. If he wants a surgical option, he should look into preputioplasties, which don't remove the foreskin but make it more elastic. Obviously circumcision is on the table, but it should be only a last result procedure. Most importantly, he should research his condition and investigate the various options he has.

Go to google and enter "foreskin stretching."
I seriously doubt that he needs to be circumcised. It will take patience and persistence but I think if he maintains a daily routine in stretching he will retract behind the head in a couple of weeks.
