He's a heavy chain smoker and I just wish he'd quit smoking. He said he tried quitting for a week but then went back into it.

How should I tell him without offending him?

I really do care about him and I'm worried about his health in the future.

The thing I hate most about people is when they try to tell me to stop smoking.

Let him make his own decisions, but if you're really worried the only thing you can do is tell him how you feel.
If you repeatedly say things or annoy the fuck out of him he will get pissed off.

He's been smoking for  5 years now..Geez he's such a good guy but his bad habits...

Most common things people say is " Smoking is bad for you." I mean come on, smokers know that. I really wanna tell him how I feel though.

You can't, just look at reality, in the end people do what they want no matter the outcome. He knows the facts, what more could you do? Threaten to not be his friend?
