This is serious advice: Work on an offline copy. Download Xampp and install it - it basically lets you have a webserver on your computer, so you can design and put it in that folder. The advantage of this is it lets your have mysql databases running as well (it even comes with phpMyAdmin). You can run any type of script file (.php etc) and pull stuff from your database just like you would on the actual webserver. Probably the biggest advantage to doing it this way is that you can make sure everything works perfectly, show it to the client, and then upload/update everything at once. Download the entire contents of the web server onto your computer, set up Xampp so that you're running whats on thier server on your computer, and then edit to your hearts content.

I personally tell them I require FTP access etc. I would never work through e-mail and telling them what to do etc. As for this person, I assume it is because they don't want to give you their password for the FTP account etc, I suggest you see if they can set up another ftp account for you with your own password.

Yeah, they claimed that they can't set up another FTP account because it "screws up his website." It's like, dude I'm not an idiot and know you're lying. Or, you're the idiot that doesn't know how to set up another one. Er. I'm tempted to drop the job.
