Apparently no one in my area does horizontal lip piercings around here although I found someone who does microdermals and genital piercings but no one has experience with horizontal so I'd getting a vertical labret which I know looks good on me.

They're pretty unpainful to get done the only bad thing about them is you gotta make sure the balls are always on really tight. If the ball comes off and it falls out it will close really fast. I had mine 6 months and it came out for maybe 10 minutes and I couldn't get it back in.
