Depends on the site in question. Usually, I make it first, but with a couple of my financial endeavours I've purchased the domain first.

obviously creating it is the best first step but getting a domain early before its taken is also a key....
i had a few in mind for my site and when it came to buying one the main one i wanted was taken but i were available in the end i thought playswfgames.com was allright

most hosting has cPanel which allows you to upload via the file manager hosts normally also have FTP in cPanel

You need an FTP client such as fileZilla to connect to an FTP server, really. The web based ones suck.

you still need a ftp client.
i dont have a problem with uploading via cPanel but ftp could be faster/easier

I ussually buy the domain and use my own hosting.. Then just create the files on the server so its updated in real time for me to see easily :) (PHP/MySQL etc...)
