Try writing a polite email letter to her, CC other faculty people and explain the situation, (pretend to) ask advice from everyone about what to do. Hopefully she'll feel intimidated/threatened enough to give you the stuff you need.

Many times, she just gives me a look saying "what am I meant to do about it" I'm going to try and get the sheet from other people, ask them to search their rooms for it, because that's all I can do. I'm not gambling on my future.

Have you explained to her you don't have the resources required to complete the homework?

I'm just worried if I take it to people above her then I still have to deal with her for the rest of the year. There are no other psychology teachers so what if she refuses to help me again?

Take it to department and/or head of year. You need to go above her if she's refusing to give you the correct resources.
