It means that you are the first person they believe that they could settle down with and spend the rest of their life with. The only confusing statement would be "(not like married but whatever)" which leaves an open end, you would have to elaborate with them on it as there are many different possibilities. You just need to ask yourself if you like them in the same way... Either way, I do advise you both talk about it and set things straight and take them in the direction best suited for both of you. Only problem is, your friend has some conflicts with himself, and he's letting you know. I think he is telling you this for the sake of you knowing, and so you can help him work out his problems. I think he wants to love you as well, but he may be afraid that if you two become lovers, something may happen to where you two can't be lovers anymore, and that strong friendship you two had will crumble completely.
