This is basically how I do it:

I pretend like everything is okay in front of my friends and family.

Then I have a melt down at my psychiatrist's office 3 times a week.

I randomly cancel plans with friends on days when I'm feeling particularly down.

Then I cry and moan about my problems to random people on-line.

Should I actually start telling people who may actually care?

I do that.
Apart from the psychiatric bit.
Nobody I actually know, knows about my problems

Sometimes when you tell the people you actually know, they don't know how to handle it. It's easier to tell people you don't really know because there's not much for you to lose, yet you still get it out. Hey, it works for you, right?
