Firstly, replace all drinks with water. Eat real fruit, not fruit juice or fruit chews or anything. REAL fruit. It will help with sugar cravings and give you lots of vitamins!
Also, talk to your doctor about a work out routine. I'm not sure what areas you need to focus on most. Going for 30 minute walks every day helps me a lot. Plus, it's a great way to get some alone time and gives you a chance to clear your head.
Tell your parents about your diet and lifestyle change so that they can support you. Ask them not to tempt you with things you shouldn't be eating and do your best to avoid salt and sweets. Reward yourself every meal you go without eating something unhealthy.
