If you're not a virgin, then it prob wont hurt too much, but it will be uncomfortable. I think of it this way:  Any doctor is MORE THAN HAPPY to look at a clean vagina rather than a nasty one.  Besides, this is what they went to school for 8 years for.  They know what's coming, don't feel nervous or anxious.  They probably look at 20 vaginas a day.  And some of them are probably infested with STDs.  Yours is clean, I'm sure.  You'll be fine.  Trust me.
